Xarelto (rivaroxaban) is a prescription blood thinner, according to its FDA label on DailyMed. It was one of the first 10 drugs with a Medicare negotiated price, but the CMS Drug Price Negotiation Program page lists Xarelto under drugs with deselection determinations, removed from the selected drug list effective January 1, 2027. CMS deselects a drug when a generic version is approved and marketed. The CMS negotiated price file end-dates the Xarelto negotiated price on December 31, 2026.
How it works in 2027. Without a negotiated price, each Part D plan pays its own contracted price for Xarelto, and that price drives your share. Under the 2027 standard Part D benefit in the CMS CY2027 Rate Announcement, you pay 100% of the plan price until you have paid the $700 deductible, then 25% of the plan price. Once your out-of-pocket spending on covered drugs reaches $2,400, you pay nothing more for covered drugs for the rest of 2027. Many plans swap the 25% for a flat copay on their brand tier, and some set a lower deductible.
What it costs in Idaho. CMS has not published 2027 plan prices for individual drugs, so a dollar amount per fill for Xarelto in 2027 is pending. What is published: Idaho has 8 standalone Part D plans for 2027, with premiums from $5.30 to $238 a month and deductibles from $300 to $700, per the CMS CY2027 Landscape file. Plans can also change Xarelto's tier, add prior authorization or cover only some strengths, so the same prescription can cost very different amounts from one plan to the next. If you qualify for Extra Help, the same Rate Announcement caps a brand-name drug at $14.40 per prescription in 2027.
What to do next. If you take Xarelto, check your 2027 coverage before December 7, 2026, the last day of open enrollment. Send us your drug list with doses and your pharmacy, and we price Xarelto and everything else you take against the 2027 plans we represent, including Medicare Advantage plans with drug coverage.
