Quick answer: Before any Medicare appointment with CarrieAnne, you must complete a Scope of Sales Appointment form. It's a federal CMS requirement that simply lists which topics you agree to discuss — it does not enroll you in anything or obligate you to anything. It takes about two minutes online.

What Is a Scope of Appointment — and Why Do I Have to Sign One?

A Scope of Sales Appointment form is a short document, required by CMS (the federal agency that runs Medicare), that records which Medicare topics you agree to discuss before you meet with an agent. It protects you: an agent can only talk about what you approved in advance. Signing it costs nothing and commits you to nothing.

Think of it as setting the agenda for your own meeting. If you say the appointment is about Medicare Advantage and drug plans, that's what the conversation stays about — no surprise pitches for products you never asked to hear about. That's the whole point of the rule, and honestly, it's a rule we like. It matches how we work anyway: education first, zero pressure.

One more time, because it matters: completing this form does not sign you up for a plan, does not obligate you to buy anything, and does not lock you into anything. It just lets the meeting legally happen.

How Do I Fill Out the Form?

The online form takes about two minutes. The only part that trips people up is knowing which section is yours — so here's the short version:

  • Enter your name, address, and phone number in the beneficiary section — that's you, the person on (or going on) Medicare
  • Check the topics you'd like to discuss at your appointment
  • Sign, date, and submit — you're done

What Should I Bring to My Appointment?

  • Your Medicare card (red, white, and blue — or your plan card if you're already enrolled)
  • Your prescription drug list — names and dosages of everything you take
  • Your doctor list — every provider you want to keep seeing
  • Your current policy information — so we can compare apples to apples
